Fully Insured Health Plans

Types of Fully Insured Group Health plans that we offer:

  • HMO – With an HMO (Health Maintenance Organization), you only have access to providers who are contracted (called “in-network”) with your carrier. If you see a non-network provider, your insurance plan will not cover any of the cost. You will need to have a primary care physician, and you will need to get a referral from your primary care physician to see a specialist. HMO plans typically have the lowest premiums.
  • PPO – A PPO (Preferred Provider Organization) plan has in-network providers like an HMO, but you are able to visit a non-network provider with few restrictions. PPO insurance will also pay towards visits to non-network providers, but at a lower rate. Additionally, you do not need to have a primary care physician or get a referral to see a specialist, but you may need prior authorization from your insurance carrier before certain procedures.
  • POS – A POS (Point-of-Service) plan is a combination of the HMO and PPO plans. On a POS, you would need a primary care physician like on an HMO, but you also have out-of-network coverage that operates like a PPO.
  • EPO – An EPO (Exclusive Provider Organization) only offers coverage for in-network providers. You do not need to get a referral to see a specialist but may need prior authorization from your insurance carrier before certain procedures.
  • HDHP – An HDHP (High Deductible Health Plan) is usually paired with an HSA, a tax-advantaged savings account, to offset medical expenses while maintaining premium costs. HDHP’s typically have a lower premium and higher deductible.
  • HSA – If you have a qualified HDHP, you are able to set aside money on a tax-preferred basis into an HSA (Health Savings Account.) This money can only be spent on eligible medical expenses (e.g., you can use it to pay for doctor visits or prescriptions, but not for cosmetic surgeries or nutritional supplements.) The employee and employer can contribute to this fund with a combined maximum contribution of $3,600 for individuals/$7,200 for families in 2021 and $3,650/$7,300 for 2022. Due to IRS guidelines, an HSA may only be paired with a qualified HDHP plan.
